I might have a suggestion:
Visit Prof. Jose Luis Martins' page:
http://bohr.inesc-mn.pt/~jlm/pseudo.html
and apparently can give it a try for BHS pseudopotentials
in ABINIT format. (Please try "grep -ir BHS *" in the directory
where you untar the source archive.) It seems to produce what you like ...
(N.B. I have not tried the code, so please try it at your own risk
for your purpose. You might want to ask questions to the author (if any).
if it seems to run successfully, your feedback on this ML would be highly
appreciated. Such report would be included in ~ABINIT/Infos or FAQ.)
