I have met some problem very simlar to yours. It seems correspond to the "tolmxf" parameter. If tolmxf
large than the calculated forces, the problem occures. I suggest you low down the "tolmxf" to about 10-8 (or even smaller) .My platform is also Xeon 64 with ifort 9 . Hope my suggestion work for you.
